Volume Loss & Thin Lips

A face that reads as tired when you are not. Lips that lipstick has to do the work for. Both come down to volume the face has lost, and both can be put back a little at a time.

What’s
going on?

As the face ages it loses collagen and volume, and the loss is not even. Cheeks flatten, temples and under-eyes hollow, and the lips thin and lose their border. None of it happens overnight, which is why it is usually a photo rather than a mirror that makes you notice: you look tired in every picture, and you were not tired in any of them.

The treatments here all put back what was lost with hyaluronic acid, a substance your body already produces, in a soft gel placed beneath the skin. Dermal fillers restore the cheeks, temples, under-eyes, and chin, and a supported cheek often softens the folds beside the nose and mouth below it. Lip filler shapes and fills the lips, defining the border before it adds volume. Full facial balancing plans several areas as one face.

For some people this is about shape: a top lip smaller than the bottom, or a border that has lost its edge. For others it is about restoring volume the face and lips have lost, in proportion, so the result reads as rested rather than done.

Filler is placed in small amounts, a little at a time, and the change shows before you leave. At the consultation your injector looks at where the volume has gone and tells you plainly what one syringe will and will not do. If filler will not get you the result you want, we will say so.

Volume lost with age or after losing weight, flattened cheekbones, shadows under the eyes that sleep does not fix. These point to dermal fillers, usually starting at the cheek, which lifts the mid-face and softens the folds below it.

Lips that disappear when you smile, a border that has lost its edge, or fine vertical lines that lipstick bleeds into. These point to lip filler: definition and balance first, then only as much volume as your features carry.

If the cheeks, the lips, and the chin are all on the list, full facial balancing treats the two or three areas that bring the face into proportion, usually with less product than three separate appointments.

Put back what was there

Dermal fillers do the heavy lifting for lost volume. A smooth hyaluronic acid gel is placed beneath the skin with a fine needle or a blunt cannula, in small amounts, with your injector checking symmetry as they go. On the cheekbone it restores mid-face volume and lifts, which is why the cheek is often the place to start: supporting it softens the nasolabial folds and marionette lines beneath without treating them directly. In the temples it restores the curve from brow to hairline. Under the eyes it softens the hollow and the shadow, placed conservatively because the skin there is thin.

Lip filler is the same hyaluronic acid in a soft gel, and it starts with shape rather than size. The lips people admire are defined at the border, balanced top to bottom, and even side to side, so that is where your injector begins. Volume comes after, and only as much as your face carries. Later in life the lips thin and develop fine vertical lines above them; a soft filler along the border restores a little of what was there and smooths the lines, without the overfilled look that comes from too much product placed without regard for the rest of the face.

Full facial balancing is for when the volume has gone from several places at once. Rather than treating the cheeks, then the lips, then the chin as separate appointments, your injector looks at the whole face from the front and in profile and finds the two or three placements that change how everything else reads. A chin brought forward, a cheek supported, lips sized against both. It usually means less product than treating areas one by one, because each placement does more than one job. How cheek filler restores volume and lifts explains why a plan so often begins there.

Lips, cheeks, or the whole face

The three differ mostly in time and in how long they hold. Lip filler is the quickest visit, 15 to 30 minutes, and the lips swell for the first 48 to 72 hours before the final shape shows at one to two weeks. Because the lips move constantly when you talk and eat, results last six to twelve months, and most people book a touch-up every six to nine months. Plan the appointment around any event, and expect what you see on the day to be a little more than you will keep.

Dermal fillers in the cheeks, temples, and under-eyes take 30 to 45 minutes, with a day or two of swelling and the final result once it settles within a week or two. They hold much longer: six months to two years depending on the area and product, with the cheeks and chin usually needing a top-up every 18 to 24 months. Full facial balancing runs about an hour when the plan fits one visit, and some plans are staged over two or three so each area can settle before the next is judged. In every case the filler is hyaluronic acid, which is gradually absorbed and can be dissolved if ever needed. How long dermal fillers last goes into the differences by area.

Which to choose comes down to how many areas have changed and how far you want to go. One area that bothers you points to that area's filler. Several point to balancing, and if your face is already in proportion, your injector will tell you so and send you home. The consultation is free, takes 45 minutes, and ends with one clear plan and one clear number before anything is scheduled; if you and your injector agree, treatment can follow the same day. You will be greeted by name and seen on schedule, and if filler is not going to get you the result you want, you will hear that at the consultation, not after.
